The judge has unsealed the search warrant issued against Applied Pharmacy Services in Las Vegas ... where authorities believe Dr. Conrad Murray obtained the Propofol that ultimately killed Michael Jackson.

According to the affidavit, during the search of Dr. Murray's home and office, cops recovered a sales receipt from Applied Pharmacy. The receipt showed that on May 12, 2009, Dr. Murray purchased 4 100 ml vials of Propofol. He also purchased 3 vials of benzos. One of the drugs Murray purchased -- Flumazenil -- is actually an antidote to a benzo, used to counteract the effects of benzos.
The receipt for the drugs totaled $853, with a $65 FedEx fee to overnight the drugs.
The affidavit states that cops were on the hunt for Visa and FedEx information linking Dr. Murray to the sale of Propofol by Applied Pharmacy.
The affidavit says at the scene of Michael Jackson's death at his home, Propofol was recovered "in Dr. Murray's doctor's bag and on the bedside table." The Propofol was in 20 ml and 100 ml bottles.
According to the affidavit, Dr. Murray told cops he was not the first doc to introduce Jackson to Propofol. Dr. Murray stated Dr. Arnold Klein and Dr. Allan Metzger had given Jackson medicine that was not working.
And there's this ... according to the docs, "Detectives interviewed Grace Owanda (sic), Jackson's children's nanny. Owanda stated that Jackson was currently being treated by Dr. [Arnold] Klein and the last physician to treat Jackson was Dr. Larry Koplin."

Big clue in the Michael Jackson investigation -- the LAPD detective leading the probe indicated January 18, 2010 may be a significant date.

Detective Orlando Martinez asked a Las Vegas judge this AM to keep search warrant documents related to Applied Pharmacy secret until January 18. Law enforcement sources told us previously cops didn't want the warrant made public until the investigation was completed.
As we first reported, the LAPD believes Applied Pharmacy supplied Dr. Conrad Murray with the Propofol that ultimately killed Michael Jackson. They are on the hunt to make sure other pharmacies didn't supply Propofol.

According to Janet Jackson, her family had tried several times to stage interventions before Michael died of a drug overdose -- but, obviously, they "weren't very successful."

In an interview with Robin Roberts, Janet talked about their attempts to get Michael clean, and though she says he understood "that it was out of love," she added "people tend to be in denial."
As we previously reported, Janet holds Dr. Murray responsible for killing her brother.

"Big Brother" winner Adam Jasinski is once again stuck in a small area with people he doesn't like, because the judge in his drug trial called him a flight risk today and denied his bail.
Jasinski was in federal court in Massachusetts today, after he was arrested by the DEA earlier this month while carrying 2,000 oxycodone pills as part of an alleged drug deal.
Today the federal judge decided Jasinski was a flight risk, a danger to society and had violated his probation from a 2004 marijuana arrest ... so his bail was denied.
After Jasinski was arrested, he allegedly admitted he was using his "Big Brother" winnings to buy large quantities of drugs.

He's got the tax man breathing down his back -- but there's one thing MLB bad boy Darryl Strawberry isn't worrying about: Finding the hottest club in NYC.

Strawberry -- who was in and out of jail numerous times in the last 10 years for solicitation and drug related incidents -- says "them days are over for me."
Though currently filming "Celebrity Apprentice," Darryl reportedly owes $554,000 in taxes. Maybe Trump can help him get "over" that too.

Adam "Baller" Jasinski won "Big Brother" in 2008 -- and now he'll be sharing living space with even more strangers behind bars ... because cops say he used his $500,000 winnings to fund a drug dealing business.
According to FOX 25, Jasinski was busted Saturday night by the DEA with 2,000 oxycodone pills during an attempted drug deal in North Reading, MA. Jasinki was thrown in jail and charged with possession of oxycodone pills with intent to distribute.
Cops say Jasinski admitted to funding his illegal venture with his $500,000 winnings from the show. He'll remain in police custody until his next hearing on Thursday.
Adam faces up to 20 years in the slammer and a $1 million fine.
UPDATE: Big Brother was definitely watching when Jasinski got busted -- the person he was allegedly trying to sell the pills to was a government witness who was wearing a wire.
Cops say Jasinski pulled a pill-stuffed sock from his pants and handed them over -- and that's when they swooped in for the bust.

The arresting officer also claims Jasinski did not go quietly into custody, accusing the reality star of putting up a struggle before he got Adam into a fresh pair of cuffs.

Dr. Conrad Murray's car was seen tooling around L.A. this weekend -- and while Murray wasn't in it, his ride stopped somewhere he knows all too well.

TMZ has learned one of his bodyguards has been driving the BMW to the Santa Monica apartment of Murray baby mama Nicole Alvarez -- the same pad the doc used to have Michael Jackson's Propofol delivered from a Las Vegas pharmacy.
As we previously reported, Murray was living at the apartment, when he was giving Jackson nocturnal Propofol infusions at the singer's home.

Howard K. Stern's lawyer said just because Anna Nicole Smith's former bodyguard testified Howard liquefied Valium and injected it into Anna does not make it true.

Steve Sadow said when he cross-examines "Big Moe" tomorrow, there will be plenty of reasons for "serious doubt."

Michael Jackson was reportedly in good health at the time of his death ... other than the fact that his body was riddled with injection sites and had arthritis and lung damage.
It sounds preposterous, but a new report claims the autopsy findings conclude Jackson was a fairly healthy 50-year-old and none of his health issues were life-threatening.
What doesn't make sense: Jackson was taking multiple powerful medicines -- sometimes in dangerous combination -- and the needle marks on his body underscore his addiction to anesthetics and other drugs. So how does that make him "fairly healthy?"
According to the report, Jackson -- who was 5'9" -- weighed 136 pounds at the time of his death.
Here's the underlying point -- the conclusion that Jackson was "fairly healthy" gives prosecutors ammo to prove Dr. Murray's dose of Propofol did Jackson in. But here's the problem -- when the jury hears about the injection sites and other drug and health problems, it could cast doubt on the Coroner's findings.

TMZ has learned the New York City Medical Examiner has determined the cause of death for Adam Goldstein -- DJ AM.
The M.E. says the cause of death is "acute intoxication" due to the combined effects of cocaine, oxycodone, hydrocodone (Vicodin), Ativan, Klonopin, Xanax, Benadryl and Levamisole (a drug used to cut cocaine with).
The manner of death is listed as "accident (substance abuse)."

Howard K. Stern has just been arraigned on 11 felony counts related to Anna Nicole Smith's prescription drug use.

The additional charges includes allegations Stern helped Anna score opiate prescriptions using aliases.
Dr. Khristine Eroshevich and Dr. Sandeep Kapoor were each arraigned on 6 felonies as well.
All 3 defendants just pled not guilty.

TMZ has learned prosecutors plan to throw the book at Howard K. Stern this morning.
Stern, along with two doctors, have been charged with multiple crimes for alleged prescription abuses involving Anna Nicole Smith. Stern has been charged with six felonies.
Our sources say during a court session this morning, prosecutors will seek to add five additional charges, bringing the number of felony counts against Stern to 11.
Stern insists he did nothing wrong and will fight all charges.

A pharmacist told Anna Nicole Smith the drugs her internist prescribed to cope with her son's death were "pharmaceutical suicide," according to just released documents.
And, in a stunning turn, the court papers filed by the State of California claim psychiatrist Khristine Eroshevich and internist Sandeep Kapoor -- both being prosecuted for their prescribing practices in the Smith case -- crossed the line by having sexual contact with Anna Nicole.
According to the document, obtained by the L.A. Times, three pharmacists refused to fill the prescriptions Eroshevich and Kapoor had written. One of the pharmacists claims he reacted by saying to himself, "They are going to kill her with this."
According to the documents, five days after her son, Daniel, died Eroshevich asked for two sedatives, 300 tablets of methadone, a muscle relaxer, an anti-inflammatory drug and 4 bottles of a painkiller.
As for the alleged improper sexual contact, authorities found photos of Anna Nicole and Eroshevich naked and embracing in a bathtub. As for Kapoor, authorities cite a video showing a shirtless Kapoor "kissing and nuzzling" Anna Nicole.
Ellyn Garofalo, the attorney for Dr. Kapoor, says "There's been a misreading of the medical records." She adds, "When all the facts are disclosed, they will show that Dr. Kapoor's treatment of Anna Nicole Smith was in good faith and consistent with good medical practice."

There's a new drug problem in the U.S. -- Propofol.
We've called a bunch of hospitals and anesthesiologists who tell us ... since Michael Jackson's death a number of patients were worried sick about being put under with Propofol. The docs have had to walk their patients through the procedures they use to ensure safety.
We're told some patients are outright refusing to be put under with the drug that killed M.J.
The medical experts we've spoken with say Propofol is a great drug that is relatively safe when administered properly -- that was clearly not the case with Jackson.
